All students entering 7^th grade at Lufkin Middle School must have an up-to-date shot record. **This would include the requirements for a Tdap, MCV4, and a Varicella shot for those students who have not had a case of chicken pox or two Varicella shots. In addition, a valid PPD/TB skin test with results is required within the last 12 months for all new to LISD students and all students entering 7th grade. You will not be able to pick up a schedule in August 2013, without proof of completed Immunizations. Please bring a copy of the updated shot record to school. If you take care of this now, it will be good for the start of next year.
